Created in 2023, the partnership allows Reimagine Care to offer cancer patients digital, virtual and in-home care. Patients in the program have access to a team of clinical oncology specialists 27/4, 365 days a year, via text, phone or video call.
During the first phase of the partnership, patient engagement rates rose to 98% in the first 30 days and more than 80% after 90 days. Reimagine Care managed and resolved 87% of patient symptoms without the need to escalate to Memorial Hermann’s facilities, and helped reduce emergency visits to less than 5% of patients. The partnership also reduced symptom-related interruptions to Memorial Hermann physicians by 87%.
The partnership is now expanding to patients and physicians across the Greater Houston area.
