Decentralizing clinical trials boosts rural patient enrollment: Mayo Clinic

By decentralizing clinical trials, Rochester, Minn.-based Mayo Clinic increased enrollment of patients from rural communities and underserved racial and ethnic populations, according to a study published June 5 in JAMA Network Open

Mayo Clinic researchers evaluated data from 7,469 patients who participated in 765 clinical trials conducted at the health system’s Rochester, Phoenix and Jacksonville, Fla., campuses between January 2024 and March 2025.

Here are three things to know from the study:

  1. Mayo Clinic decentralized clinical trial access through remote informed consent, video visits, in-home blood draws, remote monitoring and home delivery of oral medications.

  2. The program increased clinical trial enrollment of patients who lived more than 120 miles from a Mayo Clinic campus from 18.9% to 29.6%.

    Patient enrollment from rural communities increased from 16.6% to 24.9% and increased from 12.5% to 17.4% among underserved racial and ethnic populations.

  3. “Researchers found the approach expanded the geographic reach of clinical trials while increasing participation among patients who have historically faced barriers to enrollment,” Mayo Clinic said in a July 20 news release. “Broader participation can help ensure clinical trial findings better reflect the patients who will ultimately receive new treatments.”
